In this case, I started detaching the flap before the extractions, because that way the roots are much more evident, facilitating their extraction.
After the extraction, I drilled the maxillary sinus with a spherical drill to locate the floor of the maxillary sinus, because that way I can orient myself three-dimensionally to the correct position for drilling the posterior implants, preventing them from being inside the maxillary sinus.
After that, I perform the anterior implants, reaching the base of the nose bone, where I achieve greater anchorage due to the increased bone density in this region.
